Geode Health is seeking a Licensed Mental Health Therapist in the Park Ridge, IL area to diagnose and treat mental health disorders and provide psychotherapy across a broad range of conditions. You will collaborate with an integrated care team to coordinate treatment, guide goal setting, and monitor outcomes using measurement-based care.

The role supports a hybrid schedule with three days onsite and two days telehealth.

Qualifications

• Active IL license (LCSW, LCPC or LMFT) in Illinois.
• Experience conducting comprehensive assessments and developing treatment plans.
• Proficiency with an EMR system and accurate electronic health records.

Responsibilities

• Diagnose and treat mental health disorders.
• Provide psychotherapy across a broad spectrum of conditions.
