The role offers a hybrid schedule with three days onsite and two days telehealth, full-time hours, supportive benefits, and a collaborative integrated care team, with a focus on measurement-based care and accessible, high-quality mental health services.

Qualifications

• Active IL license (LCSW/LCPC/LMFT) required.
• Experience conducting comprehensive assessments and developing individualized treatment plans.
• Proficiency with an EMR system and maintaining up-to-date patient records.
• Understanding of mental health disorders and treatment modalities.
• Willingness to provide care across a general spectrum of conditions.
• Preference for therapists with 1+ year psychotherapy experience.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy.
• Collaborate with healthcare professionals to coordinate care and crisis intervention if needed.
• Partner with providers to ensure full spectrum psychiatric and therapeutic care.
• Participate in a measurement-based care approach to track outcomes.
