You will join an integrated care team and help patients define goals with personalized, evidence-based treatment. The role offers a hybrid schedule with three days onsite and two days telehealth, supporting work-life flexibility while building patient caseload and delivering high-quality care.

Qualifications

• Active IL license in LCSW, LCPC, or LMFT.
• Experience delivering psychotherapy and comprehensive assessments.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy.
• Collaborate with integrated care team for care coordination.
• Maintain accurate electronic health records for all patient encounters.
• Develop individualized treatment plans and monitor progress.
