Geode Health in the greater Chicago area seeks a Licensed Mental Health Therapist to diagnose and treat a broad spectrum of conditions through psychotherapy. You will collaborate with an integrated care team to coordinate care and provide crisis intervention when needed.

The role supports a hybrid schedule with three days onsite and two days telehealth, offering flexible planning and a supportive work environment.

Qualifications

• Active license (LCSW, LCPC, or LMFT) in Illinois.
• Experience conducting comprehensive assessments and developing individualized treatment plans.
• Proficiency with EMR systems and maintaining up-to-date electronic health records.
• Desire to work with a broad spectrum of mental health conditions.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy.
