Job summary

Geode Health is expanding its team in the Chicago area with licensed mental health therapists. You will diagnose and treat mental health disorders, provide psychotherapy, and coordinate with a multidisciplinary team to ensure comprehensive care.

The role offers hybrid scheduling—three days onsite and two days telehealth—plus a strong benefits package. We value evidence-based care and outcome tracking, with CME support, medical/dental/vision coverage, and a 4% 401(k) match.

Qualifications

• Active license in Illinois: LCSW, LCPC, or LMFT.
• Experience conducting comprehensive assessments and developing individualized treatment plans.
• Proficiency with an EMR system and maintaining accurate electronic health records.

Responsibilities

• Diagnose and treat mental health disorders and provide psychotherapy across the general spectrum of care.
• Collaborate with other healthcare professionals for care coordination and crisis intervention if needed.
• Develop and implement individualized treatment plans and monitor outcomes using measurement-based care.
• Engage in hybrid scheduling with onsite and telehealth sessions to support patient access.
