Job summary

AppleGate Recovery is seeking a Full Time Licensed Counselor for MAT clinics to support adults with opioid use disorder. The counselor will provide individual and group counseling, assess eligibility, and collaborate with a multidisciplinary team to develop care plans aimed at recovery.

The role emphasizes trauma-informed practice, evidence-based methods, and aftercare planning; a strong commitment to compassionate, non-judgmental care and respect for patients is essential in this outpatient

Qualifications

Must hold an LCDAC, CADC, LCSW, CSW, LMFT, LMFTA, LPCC, or LPCA in the state of Tennessee.

Responsibilities

Provide individual and group counseling services; direct service at regularly scheduled intervals based on patient need, best practices and state regulations; coordinate all services throughout treatment.
Conducts initial assessment and evaluation to determine eligibility for services.
Complete Bio-psychosocial assessment.
Establish individualized treatment plan with each patient and review as clinically indicated.
Respond, as needed, to patient grievances and complaints.
Develop aftercare and discharge plans.
Will provide support for other clinic locations.
Other related duties as determined by supervisor.
