Job summary

AppleGate Recovery in Crestview Hills, Kentucky, seeks a compassionate, full-time Licensed Substance Abuse Counselor to support patients in Medication Assisted Treatment for opioid use disorder. The counselor will collaborate with clinicians to develop individualized care plans and provide both individual and group counseling.

Qualifications

• Must hold at least one relevant Kentucky license (LCADC, CADC, LCSW/CSW, LMFT/LMFTA, or LPCC/LPCA).
• Prior experience working with individuals affected by chemical dependency.
• Successful completion of drug screening and criminal background check.

Responsibilities

• Provide individual and group counseling services at scheduled intervals.
• Conduct initial assessments to determine eligibility for services.
• Complete bio-psychosocial assessment and develop treatment plans.
• Coordinate services with the multidisciplinary team and providers.
• Respond to patient grievances and develop aftercare/discharge plans.
• Conduct ongoing progress reviews and adjust plans as needed.
• Perform other duties as assigned by supervisor.
