Job summary

Human Touch Home Health is seeking a compassionate Licensed Master Social Worker (LMSW) to support patients and families across Northern Virginia, including Fairfax, Falls Church, Alexandria, and surrounding communities. This PRN role offers flexible scheduling, mileage reimbursement, and the opportunity to build a caseload that fits your availability while delivering patient-centered social work services.

Qualifications

• Active Virginia LMSW license and MSW degree from an accredited program.
• Valid Driver's License and reliable transportation.
• Strong assessment, communication, and documentation skills.
• Home Health experience preferred but not required.
• Experience with WellSky Home Health is a plus.
• Passion for delivering compassionate, patient-centered care.

Responsibilities

• Complete comprehensive psychosocial assessments.
• Identify emotional, social, environmental, and financial barriers affecting patient care.
• Connect patients and families with community resources and support services.
• Assist with discharge planning and long-term care planning.
• Provide counseling, education, crisis intervention, and emotional support.
• Advocate for patients while coordinating services with community agencies.
• Collaborate with physicians and the interdisciplinary care team.
• Complete timely and accurate clinical documentation using WellSky.
