Job summary

Human Touch Home Health is seeking a compassionate Licensed Master Social Worker (LMSW) to serve patients across Blacksburg and surrounding communities in the New River Valley. This PRN role offers professional independence and the chance to make a meaningful difference in patients' lives and their families.

You will perform psychosocial assessments, connect families with resources, assist with discharge and long-term planning, and collaborate with the care team to ensure patient-centered care

Qualifications

• Active Virginia LMSW license required.
• Master's Degree in Social Work from an Accredited Program.
• Valid Driver's License and Reliable Transportation.
• Strong Assessment, Communication, and Documentation Skills.
• Home Health or Healthcare Experience Preferred but Not Required.
• Compassionate, organized, and committed to delivering patient-centered care.

Responsibilities

• Complete comprehensive psychosocial assessments
• Identify emotional, social, environmental, and financial barriers affecting patient care
• Connect patients and families with community resources and support services
• Assist with discharge planning and long-term care planning
• Provide counseling, education, crisis intervention, and emotional support
• Advocate for patients and coordinate services with community agencies
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation
