Human Touch Home Health is seeking a compassionate and dedicated Licensed Master Social Worker (LMSW) to join our team serving Blacksburg, Christiansburg, Radford, and surrounding New River Valley communities. This PRN role offers flexibility and professional independence to make a meaningful difference in patients' lives.

As a Home Health LMSW, you'll conduct psychosocial assessments, connect families with community resources, assist with discharge planning, and collaborate with the care team.

Qualifications

• Active Virginia LMSW license required.
• Master's Degree in Social Work from an Accredited Program.
• Valid Driver's License and Reliable Transportation.
• Home Health or healthcare experience preferred but not required.
• Compassionate, organized, and committed to delivering patient-centered care.

Responsibilities

• Complete comprehensive psychosocial assessments
• Identify emotional, social, environmental, and financial barriers affecting patient care
• Connect patients and families with community resources and support services
• Assist with discharge planning and long-term care planning
• Provide counseling, education, crisis intervention, and emotional support
• Advocate for patients and coordinate services with community agencies
• Collaborate with physicians and the interdisciplinary care team
• Complete timely and accurate clinical documentation
