Job summary

Outfield Healthcare Partners is seeking a qualified social worker to foster a supportive environment for residents in a skilled nursing setting. You will assess social, emotional, and psychosocial needs, document thoroughly, and help residents access services in line with NASW ethics and applicable laws.

The role emphasizes collaboration with the care team to develop care plans that respect resident autonomy and dignity while addressing complex psychosocial needs.

Qualifications

• Must be certified, licensed, or registered in the state of practice.
• Bachelor’s Degree in Social Work or Human Services field.
• One year of supervised social work experience in a healthcare setting with geriatric individuals.
• Minimum of 2 years of nursing experience in a Skilled Nursing Facility preferred.

Responsibilities

• Completes social history and psychosocial assessment for each resident to identify needs and guide the care plan.
• Maintains accurate documentation compliant with federal/state regulations and center policies.
• Supports residents' right to self-determination and informs on therapeutic alternatives when needed.
• Participates in developing an interdisciplinary care plan addressing psychosocial needs and goals.
