Job summary

Team Select Home Care in Denver is seeking a Clinical Support Specialist (CSS), a licensed professional nurse who will support clinical operations in your assigned office/market. The CSS reports to the DON/DPCS/CS and ensures compliance with state, federal and accreditation guidelines.

Daily duties include auditing medical records, tracking orders, gathering Face to Face verification, care coordination, and education for field staff.

Qualifications

• QA audits of medical records to ensure proper documentation and compliance.
• Understanding HIPAA guidelines and confidentiality requirements.
• Proficient with computer systems and back-office software.
• Strong verbal and written communication across teams.

Responsibilities

• Complete quality audits of visit notes and OASIS documents for accuracy and reimbursement.
• Assist with order tracking, including 485 forms and follow-up.
• Assist with Face to Face gathering and physician verification processes.
• Coordinate care and document activities across the team and field staff.
• Educate staff on clinical compliance and best practices; maintain confidentiality.
