Job summary

North Mississippi State Hospital's Community Support Services seeks a qualified professional to obtain and maintain CSS certification, oversee client case loads, and provide comprehensive support, including housing, benefits, and medication management. Home visits and advocacy are essential parts of the role.

The candidate will conduct ongoing assessments, develop treatment plans, coordinate with external agencies, document in EMR, and participate in meetings and staff trainings.

Qualifications

  • Ability to relate effectively to severely mentally ill individuals.
  • Must have transportation, valid driver’s license, automobile insurance, and a safe driving record.
  • Must have, and maintain, a Communicare insurable driving record.

Responsibilities

  • Obtain and maintain proper certification to perform CSS, completing required annual training and submitting applications independently and timely.
  • Meet productivity requirements by planning ahead, developing a schedule, and seeking support when deficiencies are anticipated.
  • Provide case management services for clients who meet the criteria for Community Support Services.
  • Provide ongoing assessment of clients’ needs and develop appropriate treatment planning.
  • Coordinate case management and mental health center services with services provided by other agencies.
  • Provide supportive counseling to clients and their families as needed.
  • Assist CSS clients in making applications for benefits and assistance, housing, and medications management.
  • Ensure access to mental health and other community services; conduct home visits as needed.
  • Provide advocacy and family counseling/education concerning mental illness to CSS clients.

Skills

Education

Bachelor’s degree in social work, counseling, psychology, or a related field
