Job summary

Team Select Home Care in Denver, CO is seeking a Clinical Support Specialist to support clinical operations and ensure accurate documentation and compliance across visits. The CSS reports to the DON/DPCS/CS and works within state and federal guidelines.

You will conduct quality audits, assist with care coordination, and educate field staff on documentation, billing, and compliance while maintaining strict confidentiality.

Qualifications

• Active nursing license required.
• Minimum of one year of home health experience preferred.
• Strong communication and organizational skills.

Responsibilities

• Completes quality assurance audits of medical records to ensure proper documentation and compliance.
• Assists with order tracking, including 485 forms as needed.
• Supports Face to Face gathering and physician verification processes.
• Coordinates care and maintains up-to-date clinical documentation.
• Identifies and reports clinical compliance or billing issues to supervisor.
• Educates field staff on compliance and documentation during reviews.
• Attends in-services and continuing education as applicable.
• Maintains confidentiality of patient and agency information.
• Works with committees and assists in PI process as requested.
• Meets goals in annual performance and corporate compliance.
