FHCHS of PR, Inc. is seeking a Transition of Care Coordinator to oversee the Behavioral Health Transitions of Care Program. You will review discharge plans, monitor at-risk members, ensure timely follow-ups, and facilitate communication among providers and caregivers.

The role emphasizes person-centered, trauma-informed, culturally responsive care to support continuity of treatment and safe transitions from inpatient settings. Travel within Puerto Rico is required as needed.

Competenze

Master’s degree in social work, psychology, counseling, or related field.
Active license in relevant field.
Professional association membership (colegiación).
Minimum 2+ years of experience in health care.
Proficiency with Microsoft Office; strong communication skills; bilingual is a plus.

Mansioni

Identify patients eligible for Transition of Care program and coordinate safe discharge transitions.
Oversee staff assignments, scheduling, workflows, and program reporting.
Document Transition of Care strategies and ensure regulatory compliance and quality measures.
Assess readmission risks and coordinate transitional care planning and community referrals.
Coordinate communications among health plans, providers, and departments to ensure timely follow-ups.
Monitor program quality, HIPAA compliance, audits, and corrective actions.
Participate in interdisciplinary care team meetings and case discussions; provide timely consultations.
Travel within Puerto Rico as needed to perform duties.
