Job summary

Molina Healthcare is seeking a licensed Behavioral Health Transition of Care Coordinator to support Medicaid members discharged from psychiatric hospitals. This hybrid role combines remote work with travel to local facilities to ensure seamless discharge planning and continuity of care.

You will operate in Thurston or Mason counties, holding an active license (LICSW/LMSW/LMHC/LMFT/PhD/PsyD, etc.), and collaborate with discharge planners, hospitalists, and families.

Qualifications

• 2+ years in health care with 1+ year in discharge planning, care management or behavioral health.
• Active and unrestricted license in state of practice.
• Valid driver's license and reliable transportation.
• Knowledge of CTI or similar model.
• Background in discharge planning and/or home health.

Responsibilities

• Follows member through a 30-day program from hospital admission through transitions to other settings.
• Collaborates with discharge planner, hospitalists, outpatient providers, facility staff and family/support network.
• Ensures safe transitions with adequate caregiving and medical/medication oversight.
• Coordinates with public agencies and service providers for safe transition.
• Conducts face-to-face hospital visits and home visits for high-risk members post-discharge.
• Coordinates care and reassesses needs using the Coleman Care Transition model.
• Educates on seven Transition of Care Pillars: meds, health records, follow-up, warning signs, nutrition, functional needs, home/community services.
• Uses motivational interviewing and Molina guides to educate, support and motivate change.
• Assesses barriers to care and assists members to address concerns.
• Facilitates ICT meetings and interdisciplinary collaboration.
• Provides consultation to non-behavioral health managers.
• 40-50% local travel may be required.
