Mental Health Resources, Inc. is seeking an Assertive Community Treatment (ACT) Case Manager in Minneapolis, MN.

This full-time role supports individuals with serious mental illness in the community, aiming to stabilize conditions and promote recovery through coordinated, person-centered services. The ACT team operates as a "hospital without walls" and emphasizes teamwork, evidence-based practice, and outreach.

Formación

• Bachelor's degree in social work, psychology or human services related field.

Responsabilidades

• Work as part of the ACT team to develop individualized service plans promoting community living and independence.
• Monitor and support medication adherence and collaborate with clients on treatment goals.
• Provide education and collaboration with families and significant others as requested by the client.
• Advocate for clients to protect rights, privacy, and access to complaint/grievance procedures.
• Ensure all team communications and documentation are timely and compliant with policies.
