POSITION TITLE : Licensed Care Coordinator

PLACEMENT : Professional, Grade S09

HIRING RANGE : $59,142 - $66,535 Annually

POSITION SUMMARY

Under the direct supervision of the Director of Center for Student Wellness and Support, the Licensed Care Coordinator will serve as a bridge to appropriate care at JJC or in the community by supporting and working with students at-risk with mental health crisis and providing social support needs through interventions and connection to follow-up services and off-campus referrals. Serve as a member of the Behavioral Intervention Team (BIT), overseeing CARE REPORTS, and assisting with early intervention efforts for students exhibiting concerning behaviors. Respond to student crises on campus, providing immediate support and coordination of care. Assist in the daily operations of the JJC food pantry, ensuring equitable access to food and hygiene products for students in need. Develops and maintains a database of referral sources and community services; documents and establishes systems for receiving, managing and tracking student BIT cases; tracks and reports students' progress and other care developments. Follows up on best course of action for student success. Flexibility to adapt to the occasional evenings or weekends, and travel to extended and off-campus facilities is required. Position requires participation in crisis support (full week at a time) to help manage after-hour crisis.

ESSENTIAL JOB DUTIES AND KEY RESPONSIBILITIES

1. Meet with individual students to perform clinical assessments, crisis assessments, and care coordination assessments, and develop an immediate action plan, which includes individual counseling, care coordination, or referral to on- and off- campus support.

2.Track and follow-up on students' completion of care coordinated action plan and BIT referrals.

3. Develop and maintain a network of on-campus support, community-based agencies, programs, and workshops that support student success, including but not limited to local / regional mental health and social services agencies, shelter and emergency housing, medical and insurance assistance, food pantries, and other social supports.

4. Collaborate and implement ongoing programming, workshops, groups, educational outreaches relating to emotional wellness, and work towards the development and maintaining of Center for Student Wellness and Support procedures and protocols.

5. Provide consultation and training for faculty and staff, as needed, to increase awareness on how to manage a student in distress and other topics relating to mental health advocacy.

6. Participate in responding to immediate students' crisis by traveling to student crisis on campus, providing immediate assessment of crisis, enacting crisis management protocols, and participate in after-hour procedures for crisis management.

7. Collaborate with Director to conduct assessment and prepare monthly / semester / annual reports on care coordination and BIT referral activities, including but not limited to trends, data analysis, and center evaluations.

8. Assist in the daily operations of the JJC food pantry by ensuring equitable access to essential resources, developing outreach strategies to increase awareness and usage, and collaborating with community partners to secure additional support for students facing food or housing insecurity.

9. Identify and participate in on-going professional development, maintain compliance with licensure requirements, and complete any mandated trainings identified by the college. Perform related duties as assigned, in compliance with college policy and procedure.

MINIMUM QUALIFICATIONS

1. Master's degree in psychology, Licensed social work, or clinical / mental health counseling.

2. Licensed Clinical Counselor (LPC), Licensed Clinical Professional Counselor (LCPC), Licensed Clinical Social Worker (LCSW) or equivalent license in the State of Illinois.

3. Two (2) years of experience working with clients / students in conducting case management and / or crisis intervention in a support team approach environment.

4. Knowledge of related sexual violence, domestic violence, mental health, and disability laws.

5. Excellent written, oral, and interpersonal communication skills and ability to work as a part of a team.

6. Demonstrate an interest in working with college students and must have sensitivity to and understanding of the needs of first-generation, low-income, ethnic minority students, LGBTQ, and students with disabilities.

7. Demonstrated ability to interpret a variety of instructions furnished in written, oral, diagram, or schedule form.

8. Proficient in Word, Outlook and Excel with ability to learn specific software such as Maxient, Titanium, and Ellucian Colleague.

9. Ability to establish and maintain cooperative and effective working relationships with other members of the college and community, displaying cultural competence as well as emotional intelligence.

10. Demonstrated commitment to the college's core values of respect and inclusion, sustainability, integrity, collaboration, humor and well-being, innovation and quality.

PREFERRED QUALIFICATIONS

1. Experience at the community college or university counseling center.

2. Experience working with more severe psychopathology.

3. Bilingual licensed therapist highly encouraged to apply

4. Working knowledge of social service agencies and Medicaid eligibility and enrollment.

5. Experience in providing counseling / case management services via remote methods, such as video or phone.

6. Focus on being student-centered with a passion for student life and associated best practices in the context of higher education.

7. Maintains own professional liability / malpractice insurance.

8. English and Spanish verbal and written communication proficiency.

9. Demonstrated multicultural competence.

PHYSICAL DEMANDS

1. Normal office physical demands.

2. Ability to travel and work between campus locations.

3. Ability to work evening hours as well as be available for after-hour crisis as needed.
