Applicants for this role have the flexibility to work remotely from their home anywhere in the Central time zone.

Position Purpose

Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, and continuity of care/member satisfaction is achieved. Interacts with members by performing outreach via phone or home visits and documents the care/services plan.

  • Provides outreach to members via phone or home visits to discuss care plans, next steps, resources, questions or concerns, and ongoing education.
  • Coordinates care activities based on the care plan and works with healthcare and community providers, partners, and members/caregivers to accommodate changes or progress.
  • Serves as support on member and provider inquiries or concerns related to care plans.
  • Communicates with care managers, practitioners, and others to facilitate member services and ensure continuity of care.
  • May support performing service assessments/screenings and documenting member’s care needs.
  • Supports documenting and maintaining member records in accordance with regulations and sharing with providers.
  • Follows standards of practice and policies compliant with contractual and regulatory guidelines.
  • Identifies needs and makes referrals to Care Managers, community organizations, and Disease Managers.
  • Provides education on benefits and resources available.
  • Performs other duties as assigned.
  • Complies with all policies and standards.
Education/Experience
  • High School diploma or GED required.
  • 1–2 years of related experience required.
Pay Range

$17.50 - $27.50 per hour.
