Applicants for this role have the flexibility to work remotely from their home anywhere in the Central time zone.
Supports care management activities and the teams assigned to members to ensure services are delivered by healthcare providers and partners, and continuity of care/member satisfaction is achieved. Interacts with members by performing outreach via phone or home visits and documents the care/services plan.
$17.50 - $27.50 per hour.

Actual pay will be based on skills, experience, education, and other factors. Total compensation may include incentives.
