Job summary

Carle Health is seeking a Licensed Behavioral Health Professional to provide assessment, intervention and consultation for individuals and groups with behavioral health needs under the supervision of the Community Based Therapy Supervisor.

Responsibilities include crisis evaluation, treatment planning with patients, coordination with emergency services and community agencies, and maintaining compliant documentation under applicable regulations.

Qualifications

• Licensed professional licenses (LCPC/LCSW/LMFT/Clinical Psychologist) per Illinois IDFPR.
• Master’s degree in social work, psychology, counseling, or related field.

Responsibilities

• Performs diagnostic assessments and develops treatment plans.
• Provides behavioral health therapy to individuals, families, and/or groups.
• Coordinates with Emergency Response Service and crisis teams for high-risk cases.
• Maintains records per UnityPlace, state, federal and accreditation requirements.
• Acts as liaison between community and patient needs, coordinating with community agencies and care providers.
