Job summary

Carle Health seeks a Licensed Behavioral Health Professional to provide behavioral health assessment, intervention, and consultation to individuals and groups with behavioral health needs. Under supervision, you will plan treatment modalities and coordinate and follow treatment plans, and provide crisis evaluation and intervention as needed.

The LBHP is recognized as a Licensed Practitioner of the Healing Arts under Medicaid Community-Based Mental Health Services Rule 140 and requires a Master’s

Qualifications

• Master's Degree in Social Work, Psychology, Counseling, or related field.
• 2 years of behavioral health experience.
• Licensure as LCPC, LCSW, LMFT, or Licensed Clinical Psychologist as per IDFPR.

Responsibilities

• Performs diagnostic assessments, collects diagnostic information, and determines immediate treatment plan needs with patients.
• Completes treatment plan with patient, identifying goals/objectives linked to assessment.
• Provides behavioral health therapy to individuals, families and/or groups.
• Coordinates with Emergency Response Service and crisis teams for high-risk clients.
• Maintains records per UnityPlace policies and regulatory requirements.
• Acts as liaison between community and patient needs; coordinates treatment goals with community agencies and providers.
