Job summary

Wilson Medical Center is seeking a compassionate Licensed Practical Nurse (LPN) for a 5x8+ evenings (3p-11p) rotating weekends/holidays, in-person care in a 90-bed Skilled Nursing & Rehabilitation Unit. The nurse will assess patients, initiate interventions, and document care plans with the interdisciplinary team.

Qualifications

• State Licensed Practical Nurse with active license in North Carolina.
• Graduate from an accredited nursing program and board-eligible/licensed.
• Ability to perform patient assessments and document care plans accurately.

Responsibilities

• Perform patient assessments and identify care needs.
• Initiate appropriate nursing interventions and monitor responses.
• Provide age-appropriate care and documentation of medical necessity.
• Maintain and revise the Interdisciplinary Care Plan for each patient.
• Complete timely QI assessments and ensure accurate records.

More about our team

Join our team in a 90-bed Skilled Nursing & Rehabilitation Unit, with 20 Rehab beds and 70 LTC beds. Staffed with Registered Nurses (RNs), Licensed Practical Nurses (LPNS) Certified Nursing Assistants (CNAs), and Medication Aides. Operating with a patient ratio of 1:15 days, 1:19 evenings and 1:25 nights, our unit cares for patients with Congestive Heart Failure (CHF), COPD, Diabetes, ESRD, Renal Failure, Dementia, Stroke, and Heart Disease. This role offers an opportunity to work collaboratively in a supportive environment focused on providing quality care and support to individuals requiring temporary rehab assistance and LTC needs.

How you'll contribute

A Licensed Practical Nurse (LPN), who excels in this role:

• Accurately performs patient assessments and identifies patient needs
• Identifies and initiates appropriate nursing interventions
• Provides care appropriate to condition and age of the patient
• Performs timely and appropriate documentation relating to medical necessity in the medical record
• Responsible for completion and revision of the Interdisciplinary Care Plan for each patient
• Performs timely and accurate QI assessments

What we're looking for- a compassionate nurse with a strong foundation in providing holistic and compassionate long-term care, with a focus on detailed patient assessment, medication administration, and chronic disease management.

Applicants should have a current state LPN license and possess a degree from an accredited nursing school. Additional requirements include:

• Basic Life Support certification is required within 30 days of hire.
• Long-term care (LTC) preferred or desired to work with LTC patients
