Job summary

Carbondale Rehabilitation and Healthcare Center LLC is seeking a dedicated Licensed Practical Nurse (LPN) to join our 3rd shift team (11 PM–7 AM). This full-time role features every-other-weekend scheduling and offers a supportive environment where your expertise is valued and your contributions matter each night.

You will deliver direct nursing care, administer medications, monitor residents, document clearly, and mentor CNAs to ensure consistent, high‑quality care while upholding infection

Qualifications

• Active, unrestricted Pennsylvania LPN license.
• Genuine passion for geriatric and rehabilitative nursing.
• Strong clinical skills and team-first mindset.

Responsibilities

• Deliver direct nursing care according to individualized care plans.
• Administer medications and treatments accurately and safely.
• Closely monitor residents, recognize changes, and communicate with RNs/physicians.
• Maintain clear, timely, and accurate documentation.
• Guide and support CNAs to ensure consistent, excellent care.
• Assist with admissions, transfers, and discharges.
• Build trusting relationships with residents and families.
• Uphold infection control and resident safety standards.
