Job summary

Carbondale Rehabilitation and Healthcare Center is seeking an LPN for the 2nd shift (3 PM–11 PM), full-time, Monday–Friday. You will provide skilled, patient-centered nursing care in a supportive long-term care setting, helping residents rest safely and comfortably as part of a dedicated care team.

Qualifications

• Active, unrestricted PA LPN license.
• Geriatric and rehabilitative nursing passion.
• Strong clinical skills and attention to detail.
• Dignity, respect and person-centered care.

Responsibilities

• Deliver direct nursing care per individualized care plans.
• Administer medications and treatments accurately and safely.
• Monitor residents, recognize changes, and communicate with RNs/physicians.
• Maintain clear, timely documentation.
• Guide and support CNAs to ensure excellent care.
• Assist with admissions, transfers, and discharges.
• Build trusting relationships with residents, families, and the care team.
• Uphold infection control and resident safety standards.
