Job summary

Carbondale Rehabilitation and Healthcare Center is seeking a Licensed Practical Nurse for the 2nd shift (3 PM–11 PM), Monday–Friday, at our skilled nursing facility in Carbondale, PA. You will be a trusted clinical leader on the evening shift, delivering high-quality, person-centered care to residents.

You’ll administer medications, monitor changes, document meticulously, and mentor CNAs to ensure consistent, excellent care while upholding infection control and resident safety.

Qualifications

• Active, unrestricted Pennsylvania LPN license.
• Genuine passion for geriatric and rehabilitative nursing.
• Strong clinical skills, sharp attention to detail, and a team-first mindset.

Responsibilities

• Deliver direct nursing care according to individualized care plans.
• Administer medications and treatments accurately and safely.
• Closely monitor residents, quickly recognize changes, and communicate with RNs and physicians.
• Maintain clear, timely, and accurate documentation.
• Guide and support CNAs to ensure consistent, excellent care.
• Assist with smooth admissions, transfers, and discharges.
• Build trusting relationships with residents, families, and the care team.
• Uphold the highest standards of infection control and resident safety.
