Yale New Haven Health is seeking a qualified Advanced Practice Provider to deliver comprehensive patient assessment, triage, and medical management within an interdisciplinary team. You will collaborate with supervising physicians to develop treatment plans, educate patients, and support families throughout care.

Qualifications

• Graduate from an accredited Nurse Practitioner or an accredited Physician Assistant Program.
• Experience preferred in the given specialty/service line, but not required.
• Active PA or NP license and related certifications as applicable.

Responsabilités

• Provide assessment, triage, and therapeutic management of patient/family needs as part of an interdisciplinary team.
• Collaborate with supervising physicians, adhere to hospital policy and clinical pathways.
• Educate patients and families on care, treatment plans, and available resources.
• Offer psychosocial support to patients and families with professionalism and empathy.
