Job summary

Baptist Health in Kentucky seeks a PRN professional to coordinate and assess patients referred to intensive outpatient programming. You will determine level of care in collaboration with the psychiatrist, patient/family, and insurance verification.

The role involves coordinating outside referrals, facilitating treatment planning, and providing brief therapeutic counseling within a hospital setting. Requires a Master’s degree and Kentucky licensure (LCSW/LPCC/LMFT).

Qualifications

• Master's degree required.
• Active Kentucky License: LCSW, LPCC, or LMFT.
• 2 years of job-related experience required.
• Experience working in a hospital setting; knowledge of substance use disorder treatment; experience in group therapy preferred.

Responsibilities

• Participates in the formulation of a treatment plan, implementation and evaluation.
• Implement therapeutic, evidence-based group therapy and psychoeducational classes to meet patient needs.
• Provide brief therapeutic counseling.
• Assist patients with paperwork for FMLA and Short-term Disability to be away from work while in treatment.
• Develops and maintains professional relationships with treatment team members and community partners.

• Coordinates and facilitates outside referrals and placement if needed.

• Participates in the formulation of a treatment plan, implementation and evaluation
• Implement therapeutic, evidenced based group therapy and psychoeducational classes to meet the needs of the patients
• Provide brief therapeutic counseling
• Assist patients with paperwork needed for FMLA and Short-term Disability in order to be away from work while in treatment
• Develops and maintains professional relationships with treatment team members, hospital and medical staff and community partners

Minimum Education, Experience, Training and Licensures Required
