Responsibilities

• Develop, implement, and monitor social service programs that meet all regulatory requirements.
• Support discharge planning, resident assessments, and individualized social care plans.
• Collaborate with residents, families, healthcare providers, and community organizations.
• Coordinate department functions and manage social services staff.
• Ensure compliance, resident rights, confidentiality, and quality standards.
