Job summary

Humana Inc. seeks a Primary Care Physician Lead for its CenterWell Senior Primary Care network.

This full-time role combines leadership of physicians, APPs, and care teams in a patient-centered, value-based setting specializing in adult and geriatric care. You will oversee clinical strategy, quality initiatives, and supervisory agreements while delivering comprehensive primary care, preventive services, and guideline-based treatments across assigned panels.

Qualifications

• Current and unrestricted medical license in the state of practice.
• Graduate of accredited MD or DO program.
• Board Certification or Eligibility in Family Medicine, Internal Medicine or Geriatric Medicine.
• Minimum 5+ years' of clinical practice experience.
• 2+ years' experience leading/supervising providers and APPs.
• EMR proficiency or ability to become proficient in the existing EMR.
• Must be willing to participate in a quarterly limited on-call coverage by phone.

Responsibilities

• Provide leadership, mentorship, and clinical oversight to physicians, nurse practitioners, and physician assistants.
• Support the execution of the market's clinical strategy, quality initiatives, and Model of Care.
• Maintain required supervisory and collaborative agreements in accordance with state regulations.
• Coach and educate clinicians on best practices, quality measures, and evidence-based medicine.
• Conduct chart reviews and identify opportunities to improve quality, efficiency, and patient outcomes.
• Assist with provider recruitment, onboarding, performance management, and professional development.
• Address complex clinical, operational, and patient-related concerns.
• Identify opportunities to improve care delivery, optimize specialist networks, and manage healthcare costs.
• Participate in provider committees, quality improvement programs, and strategic initiatives.
• Serve as a clinical resource and advocate for patients, providers, and the organization.
• Provide comprehensive primary care services and manage an assigned patient panel.
• Promote preventive care, wellness, and chronic disease management.
• Order and interpret diagnostic tests and develop treatment plans.
• Maintain accurate and timely medical documentation.
• Prescribe medications and treatments consistent with evidence-based guidelines.
• Participate in backup call coverage as needed.
