Job summary

CenterWell Senior Primary Care in Louisville, KY, is seeking a Primary Care Physician Lead to guide a team of clinicians within a designated market, maintain an active patient panel, and drive clinical excellence in a value-based care environment.

You will mentor physicians, APRNs, and PAs, support quality initiatives, oversee Model of Care, manage onboarding and performance, and collaborate with AMD/CMO and operations.

Qualifications

• Current and unrestricted medical license in the state of practice.
• Graduate of an accredited MD or DO program.
• Board Certification or Eligibility in Family Medicine, Internal Medicine or Geriatric Medicine.
• Minimum 5+ years of clinical practice experience.
• 2+ years leading/supervising providers and APPs in a clinical setting.
• EMR proficiency or ability to become proficient in the existing EMR.
• Must be willing to participate in a quarterly limited on-call coverage by phone.

Responsibilities

• Provide leadership, mentorship, and clinical oversight to physicians, nurse practitioners, and physician assistants.
• Support the execution of the market's clinical strategy, quality initiatives, and Model of Care.
• Maintain required supervisory and collaborative agreements in accordance with state regulations.
• Coach and educate clinicians on best practices, quality measures, and evidence-based medicine.
• Conduct chart reviews and identify opportunities to improve quality, efficiency, and patient outcomes.
• Assist with provider recruitment, onboarding, performance management, and professional development.
• Address complex clinical, operational, and patient-related concerns.
• Identify opportunities to improve care delivery, optimize specialist networks, and manage healthcare costs.
• Participate in provider committees, quality improvement programs, and strategic initiatives.
• Serve as a clinical resource and advocate for patients, providers, and the organization.
