The Fountains at Hosanna! in Lakeville, MN, part of Ebenezer, seeks a Lead Resident Assistant to deliver personal and health-related services that enhance residents’ physical and emotional wellbeing. This role supports daily living activities and works under RN supervision to ensure compassionate care.

Qualifications

• 1–3 years of healthcare experience
• Ability to use computers, handheld devices, cell phone, and fax machines

Responsibilities

• Assist with bathing, grooming, foot and nail care, shaving, dressing, hair care, feeding, toileting, and use of assistive devices
• Monitor residents and report changes in physical or emotional condition to RN or supervisor
• Administer or assist with medications under supervision of a Registered Nurse
• Take and record vital signs (temperature, blood pressure, pulse, respiration)
• Assist with daily living tasks like laundry, meal service, and activities
• Communicate observations to staff and contribute input to care planning
• Support orientation and training for new Resident Assistants
• Participate in staffing coordination and call-ins; cover after-hours/weekend needs
• Maintain resident safety and dignity; act as mandated reporter for potential mistreatment
• Filing, orders supplies, and coordinating physician/provider visits

Job Overview

The Fountains at Hosanna! is conveniently located in Lakeville, adjacent to Hosanna Church. We are less than a mile from Highway I-35 and near County 77. The Fountains offers seniors 92 Independent Living, Assisted Living, and Memory Care apartments. We are known for our exceptional staff, who create a warm, friendly, fun community.
The Lead Resident Assistant is responsible for providing personal care and designated health-related services to enhance and/or maintain the residents' physical and emotional wellbeing.
This caregiver schedule includes;

• 72 hours every two weeks ; Full Time
• Day Shift
• Every other weekend and holiday rotation

Responsibilities

Supports Residents With Daily Living Activities, Including

• Bathing
• Foot and fingernail care
• Shaving
• Dressing
• Hair care
• Feeding
• Toileting
• Assistance with hearing aids and/or glasses
• 1-2 person transfers and use of mechanical lifts

• Health-Related Support:
• Assists with treatments as outlined in individualized care plans under the supervision of a Registered Nurse (e.g., blood glucose monitoring).
• Administers or assists with medications, including insulin injections, as directed by a Registered Nurse.
• Takes and records vital signs such as temperature, blood pressure, pulse, and respiration.
• Daily Living Support:
• May assist with household tasks including laundry, cleaning, meal service, and resident activities.
• Resident Observation & Reporting:
• Monitors and reports changes in residents’ physical or emotional condition (e.g., mood, appetite, weight).
• Communicates observations to appropriate staff and contributes relevant input to care planning.
• Acts as a mandated reporter, promptly reporting any concerns related to potential mistreatment.
• Emergency Response:
• Responds appropriately to resident needs and building emergencies, following established protocols.
• Acts as Main Trainer for Resident Assistant's:
• Assists with welcoming and orienting new team members to their position and the community.
• Demonstrates proper resident care procedures.
• Assists with training and incorporating new standards.
• Offers appropriate feedback and encouragement to new employees.
• Creates a friendly and welcoming atmosphere.
• Assist the Director of Health Services with Resident Assistant Staffing Assignments & Administrative Duties:
• Helps with call-ins during the shift, adjusting schedule if necessary.
• Takes after hours and/or weekend staffing phone, per facility need.
• Medication destruction (under supervision of RN).
• Medication cycle sorting, set-up, printing MAR’s (RN still needs to check and sign-off).
• Provider visit coordination (e.g. podiatry).
• Orders supplies.
• Calls families when personal supplies are running low.
• Filing.

Required Qualifications

• One to Three Years experience
• Ability to use technology in the form of computers, hand held devices, cell phone, fax machines.

Preferred Qualifications

• Vocational/Technical Training or Associate Degree: CNA or HHA Certification
• Three to Five Years experience
