Pathfinder is seeking a Qualified Intellectual Disability Professional (QIDP) in Wabash, IN. The role focuses on guiding person-centered planning, developing ISPs, and supporting individuals to live, learn, and work successfully within their communities.

You'll collaborate with families, DSPs, Group Home Managers, and IDTs, lead training, monitor progress toward goals, and ensure compliance with state and federal guidelines, including ISDH rules.

Qualifications

• Bachelor’s degree in a developmental human service or therapy field.
• Minimum of two years of experience in the Human Service field.
• Proven team management skills.
• Ability to interpret financials and work with stakeholders.
• Crisis prevention, intervention, and teaching skills.

Responsabilités

• Coordinate and oversee Individual Support Plans to ensure person-centered goals.
• Develop Behavior Support Plans and obtain Human Rights Committee approval when needed.
• Lead and train Direct Support Professionals and collaborate with IDTs.
• Maintain documentation, participate in ISP meetings, and ensure regulatory compliance.
• Support individuals in living, learning, and working within their communities.
