Qualifications

• Able to exert 50-100 pounds of force to help with resident transfers.
• Two years relevant experience required, preferably in long term care.
• Current state license as a Physical Therapist.

Responsibilities

• Provides direct patient/resident treatment in accordance with an established plan of care.
• Screens and evaluates patients/residents and develops appropriate care plans.
• Offers consultation and training to patients, residents, families, and caregivers.
• Documents activities according to the facility’s clinical documentation system.
