Job summary

Qualifications

• Bachelor's degree in Physical Therapy from an accredited program.
• Current state license as a Physical Therapist.
• Two years relevant experience, preferably in long term care.
• Excellent communication skills, verbal and written.
• Genuine desire to work with the elderly and/or disabled.

Responsibilities

• Under physician's order, provide direct patient/resident treatment per care plan.
• Screens and evaluates patients/residents and develops care plans.
• Monitor efficiencies with supervisor; contribute to program development.
• Provide training to staff and participate in orientation and census/marketing activities.
• Document activities timely in the facility's clinical documentation system.
• Communicate with patients, families, caregivers, team members, and supervisors.
