Qualifications

• Preferred: Driver’s license and reliable transportation

Responsibilities

• Provides direct patient care and evaluates functional needs.
• Reviews and adjusts care plans to optimize outcomes.
• Designs and implements a plan of care based on assessments.
• Conducts therapy within defined standards of care.
• Directs support staff to achieve patient goals.
• Communicates with other professionals to coordinate care.
• Maintains policy and infection control standards.
• Documents evaluations, goals, and plans with signatures.
• Supports professional growth through education and workshops.
• Leads staff and promotes physical therapy within the community.
• Educates on insurance requirements to optimize documentation and care.
• Collaborates with director on clinical issues and patient concerns.
• Partners with front office and billing to navigate insurance issues.
