Job summary

Golden Valley Health Centers Central Valley PACE Center in Modesto is seeking a Part Time PACE Personal Care Assistant II for the Day Center. You will lead PCA tasks, support nursing staff, and deliver direct personal care to participants in a multidisciplinary team.

Schedule is Monday–Friday, 10:00am–2:00pm. Compensation is $24.31–$25.53 per hour, with duties including ADLs, documentation in the EHR, and interaction with participants and families to ensure high-quality care.

Qualifications

• High School Diploma or equivalent required.
• Current CA CNA, HHA, EMT/paramedic license or certification as clinical medical assistant.
• BLS CPR card from American Heart Association required.
• Prior PCA or caregiver experience preferred.

Responsibilities

• Communicate patient care needs to LVN/RN and document patient activities.
• Provide direct personal care and assist with activities of daily living.
• Assist training of new staff and participate in IDT meetings.
• Maintain clean patient areas and assist with intake and reception duties.
• Enter participant data into the EHR and assist with clerical tasks.
