Job summary

Central Valley PACE seeks a PACE Personal Care Assistant II for the Stockton Day Center. This role delivers direct patient care, assists nursing staff, and supports daily living activities for geriatric participants at the Stockton site, with initial training in Modesto.

The position requires a CA license/certification as CNA/HHA/EMT or clinical medical assisting, CPR certification, and ability to lift up to 30 pounds. Work is Monday–Friday, 8:30am–5:00pm.

Qualifications

• Must be able to lift up to 30 pounds.
• Current CA driver’s license with acceptable driving record and vehicle insurance.
• Certification/licensure as CNA, HHA, EMT, or paramedic per position requirements.

Responsibilities

• Communicates patient care needs to LVN and RN.
• Provides direct personal care to participants.
• Assists in training new staff.
• Performs ADL/IADL assistance and companionship.
• Maintains clean and orderly patient areas and documents care in the EHR.
