Job summary

Golden Valley Health Centers Central Valley PACE Center in Modesto is seeking a Home Care Personal Care Assistant to support nursing staff with patient care activities. This full-time role serves Stanislaus County clients, Monday–Friday, 8:00am–5:00pm, with occasional weekend flexibility.

Responsibilities include assisting with daily living activities, communicating needs to the supervising RN, delivering DME, and participating in care planning.

Qualifications

• Valid California Driver’s License, acceptable driving record and vehicle insurance.
• Must have reliable transportation.
• Must have to treat people of all ages including geriatrics.
• Meet a standardized set of competencies for the specific position description established by Central Valley PACE and approved by CMS before working independently.

Responsibilities

• Provides basic personal care assistance and assistance with any other activities of daily living across settings to assigned participants.
• Communicates patient care needs to the LVN/Supervising RN and update Manager.
• Deliver DME to PRT’s home and document in EMR.
• Flexible to work weekends when needed.
• Attends staff meetings and takes part in participant care planning.
