Job summary

Central Valley PACE in Modesto, CA seeks a Personal Care Assistant II for the Day Center to support nursing staff and lead PCA activities. The role involves direct patient care, ADL assistance, and collaboration within the interdisciplinary team.

Schedule is Monday–Friday, 7:30am–4:30pm. Compensation ranges from $24.31 to $25.53 per hour, with robust benefits including health, dental, vision, and retirement options. Must hold CA CNA/HHA/EMT/Paramedic licensure and CPR certification.

Qualifications

• High School Diploma or equivalent.
• Current unencumbered CA licensure as a CNA, Home Health Aide, EMT, Advanced EMT or Paramedic; or certification from an accredited program.
• Current BLS CPR Card certified by the American Heart Association.
• Prior personal care assistance experience, or ability to pass internal competencies within six months of hire.

Responsibilities

• Communicates patient care needs to the LVN and supervising RN.
• Provides direct personal care to participants as directed or assigned.
• Assists in training new hire PCA’s and other staff as needed or directed by management.
• Provides basic personal care assistance and assistance with activities of daily living, including showering, toileting, laundry, feeding and companionship.
• Supports ADLs and IADLs, transfers, companionship to appointments, etc.
• Maintains clean and orderly patient areas and assists with meal/snack service in Day Center.
• Documents treatments/procedures, observations and nursing care on appropriate forms.
• Participates in teaching/learning with others and adheres to policies.
