Job summary

Central Valley PACE in Modesto, CA, seeks a Personal Care Assistant to support nursing staff with patient care activities in a home care setting. Schedule Monday–Friday, 8:00am–5:00pm, with travel across Stanislaus County.

Responsibilities include basic personal care, communicating needs to the LVN/Supervising RN, and assisting with Day Center activities as needed. Valid CA driver’s license and ability to lift up to 30 lbs required.

Qualifications

• Must have a valid California driver’s license and reliable transportation.
• Must be able to lift up to 30 pounds.
• Experience in personal care or ability to pass internal competencies within six months of hire.

Responsibilities

• Provides basic personal care assistance and assistance with activities of daily living.
• Communicates patient care needs to the LVN/Supervising RN and updates Manager.
• May assist in Day Center and Activities when there is down time.
• Deliver DME to PRT’s home and document in EMR.
• Flexible to work weekends when needed.
• Home Care Assessments.
• Communicates effectively with participants, families, staff, other departments and agencies.
