Job summary

ChenMed is seeking a lead Primary Care Physician to supervise and train PCPs, maximize clinical quality and efficiency, and provide direct patient care as needed. The role focuses on PCP performance, engagement, and building a strong clinical-operations culture to coordinate care for patients with acute and complex chronic health needs.

You will lead care teams and participate in center management, while recruiting patients and interviewing providers.

Responsibilities

• Supervise and train primary care providers (PCPs)
• Maximize core model execution, including clinical quality, efficiency, outcomes, and clinician/patient satisfaction
• Provide direct patient care as a PCP and/or other clinical duties
• Focus on PCP performance, engagement, and building clinical-operations synergy and culture
• Assess, diagnose, treat, manage, educate, promote health, and coordinate care for patients with acute and complex chronic health needs
• Lead care teams
• Engage with transitional care teams and other resources for patients unable to come to the office
• Lead Super Huddle (SH) and Transforming Care Meeting (TCM) weekly
• Play an active role in center management and cover for other providers
• Recruit patients and interview additional providers
