Job summary

TeamCare Medical is seeking a Clinical Lead - Nurse Practitioner to provide executive oversight of patient care within our primary care teams. The NP will conduct histories, exams, and diagnostic testing, diagnose and treat common conditions, manage chronic illnesses, and tailor treatment plans for diverse adult patients.

The role requires NY licensure as an NP and RN, DEA certification, and strong collaboration with physicians and care teams to ensure safe, high-quality outcomes.

Qualifications

• Graduated from a nurse practitioner education program acceptable to NYSED or certified as a nurse practitioner by a national certifying organization acceptable to NYSED.
• Licensed to practice as a Nurse Practitioner in NY and as a Registered Nurse in NY; DEA Certification required.

Responsibilities

• See adult patients and provide care across a diverse community.
• Represent or serve as Medical Director in site duties when directed by Medical Director.
• Conduct annual wellness assessments and educate patients on comorbidities and disease management.
• Coordinate care plans, oversee other disciplines, and supervise implementation of care plans.
• Document accurately in the Athena EHR with proper diagnosis codes on the day of visit.
