Job summary

TeamCare Medical seeks a Regional Clinical Lead - Nurse Practitioner to provide advanced practice and clinical leadership across designated NY sites. You will perform comprehensive assessments, manage chronic illnesses, and coordinate care using Athena EHR while collaborating with physicians and regional leadership.

Travel among Manhattan/Bronx and Manhattan/Brooklyn sites is required, with 40-hour weeks and regular telemedicine support.

Qualifications

Licensed NP in NY required.
RN license in NY required.
DEA registration required.
AANP or ANCC Board Certification required.

Responsibilities

Provide NP and clinical leadership coverage across TeamCare sites in the designated region.
Travel regularly among sites and adjust work location based on needs.
Support continuity of clinical operations and coverage for vacancies, leaves and scheduling gaps.
Represent or support Medical Director functions as needed.
Collaborate with site leaders to promote consistent clinical practices and handoffs.
