Job summary

TeamCare Medical is seeking a Regional Clinical Lead - Nurse Practitioner to provide advanced clinical leadership and oversee participant care across multiple sites in the New York region. The role involves travel among sites to ensure continuity of care and to cover vacancies and scheduling gaps, while collaborating with Medical Directors and interdisciplinary teams.

The ideal candidate will hold NYSED-approved NP credentials with at least two years of primary care or home care experience, and

Qualifications

• NP license in New York State
• 2+ years clinical practice in primary care or home care
• 1+ year with adult population; otherwise training provided

Responsibilities

• Provide NP and clinical leadership coverage across TeamCare sites
• Travel regularly among sites; adjust work location as needed
• Support continuity of clinical operations and cover vacancies, leaves, and gaps
• Represent Medical Director when directed; ensure safe handoffs across sites
• Develop comprehensive diagnostic lists and ensure same-day documentation in EHR
• Oversee care plans and ensure interventions meet patient needs
