Qualifications

• Education: Nurse practitioner education program acceptable to NYSED.
• Minimum of two years in clinical nursing practice in primary care or related field.
• Licensed NP in NY, RN in NY, DEA certification, AANP or ANCC board certification.

Responsibilities

• Perform annual wellness assessments.
• Educate patients about diseases and comorbidities.
• Coordinate care with team and specialists; manage chronic conditions.
• Document patient records accurately in Athena EHR on the day of visit.
