Job summary

Community Bridges is seeking a Lead Navigator to support homeless individuals through outreach, engagement, and case management. The role requires recovery from substance use or mental health challenges and 90+ days of Navigator/Peer experience, plus driving and certification prerequisites.

The Lead Navigator will coordinate RBHA and community-based services, mentor Navigator II staff, and provide wrap-around supports for high-acuity clients in our Arizona programs.

Qualifications

• High school diploma or GED required. Associate's degree or higher in a field related to behavioral health preferred.
• 1–3 years of full‑time health‑care experience with exposure to behavioral and medical crisis situations (preferred).
• Minimum of six months of recovery from substance use and/or mental health disorders required.
• Current Arizona driver’s license and a clear 39‑month motor‑vehicle record.
• Certified Behavioral Health Technician (BHT) in accordance with A.A.C. R9‑10‑101.33 and CBI clinical policies.
• Arizona Fingerprint Clearance card (site‑specific).

Responsibilities

• Provide outreach and supportive services to help homeless individuals end homelessness.
• Coordinate and monitor RBHA and other community-based services.
• Facilitate service delivery and mentor Navigator II staff.
• Lead outreach, engagement, case management and wrap-around support for high-acuity clients.
