Job summary

Community Bridges, Inc. seeks a Lead Navigator to engage homeless individuals, manage casework, and coordinate services with RBHA and community providers. The role mentors Navigator II staff and collaborates with a multidisciplinary team to ensure seamless care.

You will oversee outreach, housing referrals, and discharge planning while supporting a continuum of care and client recovery goals within a dynamic clinic and field environment.

Qualifications

  • Candidate must be in recovery from AOD use and/or GMH for at least 1 year.
  • Minimum 90 days experience as a Navigator or Peer Support Specialist.
  • 1–3 years of health-care related work preferred; recovery-oriented service preferred.
  • Valid Arizona driver license with clean record; fingerprint clearance site-specific.

Responsibilities

  • Outreach and engagement with individuals experiencing homelessness.
  • Provide case management and wrap-around supportive services.
  • Coordinate entry, housing referral, leasing, and discharge processes.
  • Mentor Navigator II staff and collaborate with multidisciplinary team.

Job description

Lead Navigator – Community Bridges, Inc.

Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering residential, outpatient, inpatient, patient‑centered medical homes, medication‑assisted treatment, and crisis services in Arizona. The Lead Navigator role is for individuals in recovery from AOD use and/or GMH for at least 1 year, with at least 90 days of experience as a Navigator or Peer Support Specialist.

Job Summary

The Lead Navigator works with homeless individuals, providing outreach, engagement, case management, and wrap‑around services to assist them in ending homelessness. They coordinate and monitor RBHA and non‑RBHA community‑based services, support the delivery of these services, mentor Navigator II staff, and coordinate care with multidisciplinary team members.

Responsibilities
  • Conduct outreach and engagement with individuals experiencing homelessness.
  • Provide case management and wrap‑around supportive services.
  • Coordinate coordinated entry, housing referral, leasing, and discharge processes.
  • Serve as mentor to Navigator II staff.
  • Collaborate with multidisciplinary team (patient care techs, RN, counselors, clinical leads) and community providers.
Skills & Qualifications
  • High school diploma or GED required; associate degree or higher in behavioral health preferred.
  • Recovery from AOD use and/or GMH for at least 1 year.
  • 90 days experience as a Navigator or Peer Support Specialist.
  • 1–3 years of full‑time health‑care related work preferred; 1–3 years substance abuse/mental health recovery‑oriented service preferred.
  • Current Arizona driver‑license, valid and in good standing.
  • Clear 39‑month motor‑vehicle record.
  • Behavioral Health Technician credential per A.A.C. R9‑10‑115 and CBI policies.
  • Arizona Fingerprint Clearance card site‑specific.
