Community Bridges, Inc. seeks a Lead Navigator to support individuals experiencing homelessness through outreach, case management, and wrap-around services. The role coordinates care with RBHA and non-RBHA providers and serves as a bridge between the patient and community resources.

The Lead Navigator mentors Navigator II staff and oversees intake, housing referrals, and discharge planning. The candidate will engage with high-acuity clients, perform outreach, and work within a multidisciplinary

Formación

• Highschool diploma or GED required; associates preferred.
• 1–3 years health care experience preferred but not required.
• Or an equivalent combination of education and experience.
• Current AZ Driver's License is required.
• Clear 39-month Motor Vehicle Record is required.
• Behavioral Health Technician (BHT) certification per policy.
• Arizona Fingerprint Clearance card is required.

Responsabilidades

• Outreach and supportive services to assist individuals experiencing homelessness.
• Coordinate entry, housing referrals, leasing up, wrap around services, and discharge.
• Mentor Navigator II to onboard and guide program requirements.
• Coordinate and monitor RBHA and non-RBHA community-based services.
