Job summary

Southern Oaks Post Acute is seeking a dedicated LPN/LVN supervisor to direct daily nursing operations in a long-term care facility. You will lead nursing assistants, ensure compliance with policies, and participate in quality assurance and discharge planning efforts.

Strong leadership, organizational, and communication skills are essential. Your role includes directing shifts, coordinating resident care plans, and maintaining documentation with a focus on safe, compassionate care for residents.

Qualifications

• Must possess a Nursing Degree from an accredited college or university.
• Must possess a current, unencumbered, active license to practice as an LPN/LVN in this state.
• Must be able to read, write, speak, and understand the English language.
• Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
• Must have patience, tact, a cheerful disposition and enthusiasm, as well as the willingness to handle difficult residents.
• Must be willing to plan, organize, develop, implement, and interpret programs/goals/policies/procedures for providing quality care.

Responsibilities

• Direct the day to day functions of the nursing assistants in accordance with current rules and guidelines.
• Ensure nursing personnel comply with policies and procedures established by the facility.
• Participate in the development, maintenance, and implementation of the facility’s quality assurance program.
• Meet with shift nursing staff to plan services, programs, and activities.
• Greet, orient, and admit residents; coordinate discharge planning as needed.
