Job summary

Westmoreland Care & Rehabilitation Center in Gallatin, TN is hiring an LPN/LVN to provide direct nursing care to residents, including medication administration and wound care. The role supports a patient-centered environment, with a focus on safety, comfort, and dignity.

You will supervise nursing assistants, maintain timely documentation in the EHR, and collaborate with the interdisciplinary team. LPN/LVN license active in TN required; CPR/BLS preferred; experience in long-term care is valued.

Qualifications

• Current, active LPN/LVN license in the state of practice.
• CPR/BLS certification preferred (American Heart Association or American Red Cross).
• Minimum one year of clinical nursing experience; long-term care experience preferred.

Responsibilities

• Provide direct nursing care to residents, including medication administration, wound care, and skilled nursing procedures.
• Assist residents with activities of daily living (ADLs) and ensure comfort, dignity, and safety at all times.
• Supervise and direct nursing assistants and medication aides during the assigned shift in accordance with facility policies and regulations, as applicable.
• Monitor and observe residents’ physical and emotional status; notify the attending physician and responsible party of any change in condition.
• Develop, implement, and review individualized resident care plans in collaboration with the interdisciplinary team.
• Complete accurate and timely clinical documentation per facility policy and procedure.
• Coordinate resident admissions, transfers, and discharges according to established procedures.
• Participate in quality assurance activities, staff education, and regulatory surveys as needed.
• Perform other duties as assigned by the Director of Nursing or designee.
